Participating in Care4Texans

Care4Texans is a locally-owned, physician-led, clinically integrated network which empowers our provider community to collaboratively improve the way care is delivered locally.

Care4Texans Commitments:

  • Develop clinical performance standards and protocols to enhance the quality and efficiency of patient care
  • Develop an information technology platform to connect and integrate data across systems to support Population Health Management
  • Aggregate, analyze and report to its members on outcome, quality and cost of care data
  • Provide additional revenue opportunities through value-based contracts with health plans, third party payers, and government healthcare programs
  • Maintain C4T as Physician-Led organization

Provider Commitments:

  • Meet network qualification criteria: maintain a valid Texas license, DEA registration, insurance coverages and other appropriate certifications and credentials
  • Actively participate in the network and its clinical integration programs and comply with clinical protocols, criteria and procedures adopted by the Care4Texans governing body and as medically appropriate
  • Utilize an EMR and submit timely and accurate healthcare data
  • Actively collaborate and communicate with other network participants, including accepting in-network referrals for network contracted patients
  • Participation by a majority of physicians in a group
  • Participation in all upside only contracts (Can opt out of downside risk contracts)

How do I Become a Member?

If you believe in our vision and want to join Care4Texans, we welcome you to request a copy of our Participation Agreement. This agreement is required to participate in our initiaitves and to become eligible for value-based contracts that we develop with payer and employer partners.
